    Full Time: Monday - Friday Daylight

    Covering State College Area


    Hourly Pay Rate + Mileage Reimbursement


    Grane Home Health Care, one of the leading providers of innovative, versatile home health care, seeks a hands-on Certified Nursing Assistant (CNA) for an entry-level position with significant growth potential. This role requires significant direct work/contact with frail patients, so patience, compassion and flexibility are essential.

    • Visiting patients in-home and performing specified tasks outlined in personalized care plans
    • Performing simple therapeutic tasks, under the supervision of onsite physical, occupational and speech-language therapists
    • Aiding patients with personal care activities, including bathing, shampooing, skin/nail care, oral hygiene, shaving and dressing
    • Administering medications under the supervision of an onsite RN
    • Reporting observed changes in patients’ conditions and/or needs to Grane RNs or therapists
    • Ensuring a safe environment for patients and observing/reporting potential areas of risk
    • Submitting ongoing patient documentation, as instructed


    • Applicants should be at least 18 years of age and a state-certified CNA.
    • Ability to follow written and verbal instructions as well as document care provided is essential, as is basic computer/office competency.
    • All candidates are expected to understand the general hospice philosophy and be comfortable providing direct care to the terminally ill.
    • A valid driver’s license, up to date auto insurance, and reliable mode of transportation
