    Hospice On-call Registered Nurse

    Full Time- On-Call Weekends (Friday 4:30pm-Monday 8am)

     

    Grane Hospice Care is currently accepting applications for an On-call RN position from candidates with an "how can I help you?" attitude. Effective communication is key as the role focuses on building and maintaining relationships and while working accurately within established policies and procedures. A faster-than-average pace will be the norm for this position. Detail work is a major focus of the job, and those details need to be handled quickly, correctly, and efficiently.

     

    As time is usually a factor, the work must be done quickly, as well as correctly. This position requires strict adherence to guidelines and established policies, while working with and for others. Open, flowing communication is important to foster relationships with patients and their families.

     

    Candidates must be open to heavy travel and have a reliable vehicle.

    Responsibilities

    • Assuming responsibility for weekend on-call duties
    • Assuming primary responsibility for total care census, including assessing, planning, implementing and evaluating all phases of the nursing process
    • Communicating with attending physicians, Clinical Coordinator, families, caregivers, and other community resources to report changes and coordinate optimal care/use of resources for patients and families
    • Attending monthly interdisciplinary team meetings to maintain up-to-date patient records, troubleshoot problems, sets plans and action goals and work in tandem to adjust care plans
    • Obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status
    • Monitoring physical care of patients and educating family, caregivers and volunteers in proper, approved techniques

    Qualifications

    Candidates must have a current RN license to practice in Pennsylvania and a valid drivers’ license.
