    Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed.     Because of the nature of the immediate environment, extreme compassion, flexibility and an ability to quickly and successfully troubleshoot is essential, as is a strong understanding of hospice philosophy and issues of death/dying. Day-to-day responsibilities include:

    • Assessing the psychosocial status of patients, families and caregivers related to the patient’s terminal illness and immediate environment
    • Providing direct social work services such as ongoing reassessments, counseling, crisis intervention, patient/family education, discharge planning, patient/family advocacy and bereavement care
    • Maintaining clinical records via McKesson for all patients referred for social work services
    • Providing appropriate resources and external referrals, as needed
    • Serving as a liaison between patients/families/caregivers and community agencies, as needed
    • Supporting Grane’s commitment to interdisciplinary care services to ensure the highest level of patient satisfaction
    • Participating in the development of care plans and attend regularly scheduled case conferences

    Assisting physicians and other team members in understanding significant social/emotional factors related to patient health problems and death/dying


    A bachelor’s degree is social work, psychology, counseling or a related field is required, and an MSW is preferred. All candidates must have at least one year of experience in health care, hospice and/or home care, be comfortable using an online documentation/records system and possess a valid driver’s license with reliable transportation, as some home visits will be required.
