    Every other weekend: Friday 4:30p-8pm - Saturday 8am - 8apm and Sunday 8am - 8pm

    Due to growth, Grane Hospice is currently accepting applications from LPN candidates with an "how can I help you?" attitude for a Triage Nurse role. Effective communication is key as the role focuses on receiving and sending information to and from patients, families, staff, physicians, etc. A faster-than-average pace will be the norm for this position. Detail work is a major focus of the job, and those details need to be handled accurately and efficiently.

    Responsibilities

    • Make patient care decisions in response to incoming patient telephone calls, using current patient databases, and professional nursing judgment. Such responses may include giving advice or direction to the patient/family, contacting the physician, or scheduling a visit and assigning staff as appropriate
    • Triage afterhours calls with the goal of problem resolution, patient satisfaction, patient safety, symptom management, positive patient outcomes, and reduction of preventable ER/hospital admissions; this will require communication with attending physicians, other team members, and agencies when needed
    • Coordinate/direct afterhours care for the assigned home care and hospice regions including coordination of referrals, contacting patients/interdisciplinary team members as needed for staffing, scheduling of afterhours visits, DME /IV, supplies, etc.
    • Provide guidance and direction to home care/hospice providers regarding patient care and clinical issues, symptom management, patient incidents, and/or injuries
    • Review hospice referrals received after hours, assign staff as appropriate, coordinate, update and communicate any/all changes to all disciplines
    • Accurately document all communication from physicians, physician orders, clinicians, caregivers and patients, etc.
    • Document accurately and appropriately with each patient call and/or encounter in real time and maintain up to date patient records for coordination of care
    • Ensure the patients have prescribed medications and/or treatment supplies, updating files as changes occur.
    • Coordinate and assign visits identified as medically necessary or requested by hospice patients/caregiver to meet patient needs

    Qualifications

    Candidates must have a current LPN license to practice in Pennsylvania and a valid drivers’ license.
