    Grane Home Health Care, one of the leading providers of innovative, versatile home health care, seeks an RN Case Manager to plan, coordinate and provide care for a geographically-assigned caseload of patients from the day of admission through discharge. This nursing professional will carry out comprehensive physician’s instructions plus thorough care plans to ensure the highest quality of in-home care for the agency’s diverse elderly population.

    Due to the nature of this work as well as the agency’s aging clientele, the successful RN Case Manager must be caring, patient and flexible, with a proven track record of excellent patient outcomes as well as strong communication and time management skills. The Manager will be expected to be a motivated self-starter able to conduct in-depth clinical work with little or no immediate supervision. Day-to-day responsibilities include:

    • Working with elderly in-home patients within an assigned geographical territory, creating personalized home health plans that comply with federal/state regulations, reimbursement guidelines as well as Grane policies and procedures
    • Performing initial and ongoing assessments of patient needs, including Outcome and Assessment Information Sets (OASIS) at appropriate points in time
    • Initiating plans of care, regularly reevaluating patients’ skilled needs and revising recommendations, as needed
    • Implementing preventative and rehabilitative nursing procedures
    • Reporting changes in patient condition to physicians and other relevant staff, and assessing the effectiveness of existing care plans
    • Supervising, directing and assigning routine nursing visits to an LPN, in keeping with his/her scope of practice and agency policies
    • Planning discharge services in conjunction with medical personnel, families and caregivers
    • Giving skilled care and treatments to patients who require intermittent nursing services, as needed
    • Obtaining necessary supplies and equipment to provide the most effective care
    • Providing referrals, as needed


    While a B.S. in Nursing is preferred all candidates must, at minimum, have an associate’s degree, valid state RN license and current CPR certification. Candidates must also have successfully completed 1-5 years in a nursing role, with home health care experience and strong professional knowledge of all regulatory guidelines and standards preferred. OASIS-C and McKesson experience is preferred, but not required.
