Due to growth, Grane Home Health seeks a compassionate, experienced Registered Nurse to work with in-home patients. This nursing role involves a host of diverse care responsibilities, including direct work with families, caregivers and fellow medical professionals to deliver the highest quality care to the agency’s elderly population.

Due to the nature of this work, candidates must exhibit extreme empathy, strong interpersonal skills and be a topnotch communicator.


Ongoing responsibilities include:

  • Working directly with patients, as well as their families and primary caregivers
  • Providing professional nursing care based on ongoing assessments/evaluations of patients’ existing statuses and emerging needs
  • Applying preventative and therapeutic nursing procedures and techniques, and communicating all work to physicians and supervisory personnel
  • Performing specialized procedures such as IV therapy, if qualified
  • Writing and initiating plans of care, and revising as needed
  • Initiating and sustaining orders for medications and treatments, as prescribed by physicians or in existing plans of care
  • Completing, maintaining and submitted accurate and relevant clinical notes surrounding patients’ conditions and care given
  • Reporting changes in condition to physicians and other relevant care participants
  • Ensuring a safe, comfortable in-home environment for patients as well as consistent continuity of care
  • Teaching patients, families and caregivers techniques for in-home care techniques, as appropriate
  • Working in conjunction with interdisciplinary care providers and hospice teams to meet the emotional needs of patients and families
  • Supervising ancillary personnel and delegates, as required
  • Additional responsibilities as instructed or as required by patients’ emergent needs


 The successful candidate will be a motivated self-starter who can thrive in a fast-paced environment with little supervision. The Registered Nurse will be a vital piece of patients’ interdisciplinary care team and, as such, should have excellent observation and nursing skills.

Candidates must be graduates of accredited nursing schools, with active state RN licensing. All applicants should be CPR certified with at least one year’s nursing experience in a hospice, medical, surgical or other critical care environment, and should exhibit strong clinical skills as well as a professional understanding of hospice philosophies, principles and death/dying. Experience in pain management and IV skills are preferred
