Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks an entry-level Clinical Assistant to support the Clinical Coordinator and personnel in all aspects of patient care management. This is an ideal opportunity for a certified nursing or medical assistant with a desire to grow and thrive in a supportive, hands-on environment of interdisciplinary professionals.


Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. The Clinical Assistant will oversee a variety of diverse tasks and responsibilities surrounding patient care and support, including:

  • Aiding clinical team members to increase their ability to meet the complex needs of terminal patients and their families
  • Ensuring hospice aides are aware of changes in care plans by communicating and printing new, approved plans and updating care plan books quickly and accurately
  • Preparing for and coordinating clinical documentation for all to interdisciplinary team meetings
  • Faxing hospice aide schedules to facilities, as instructed
  • Tracking visit notes for completeness and carrying out additional scheduling duties, as needed
  • Organizing and tracking the paper flow for all patient documentation, from admission to discharge including physicians’ orders and critical medical information
  • Ensuring necessary signatures are obtained and paperwork processed in a timely fashion
  • Visiting various facilities at least weekly to place signed hospice documentation on facility charts after each interdisciplinary team meeting
  • Completing time, mileage and expense reports for review by the Clinical Coordinator
  • Utilizing an electronic charting system to complete core job duties, as indicated
  • Notifying team members of admissions, deaths and discharges
  • Maintaining patient weights and supply lists


To be successful in this role, candidates must exhibit strong listening and comprehension skills, and an ability to follow written and oral instructions. All applicants should be certified nursing assistants or demonstrate proficient medical assistant skills with a clear ability to handle delicate and complex situations with tact, diplomacy, attention to detail and compassion. An understanding of hospice philosophy and an ability to manage challenging situations surrounding terminal illness and death/dying is essential.


Travel is required for this position, so a valid driver’s license plus reliable mode of transportation are necessary.
