Overview

Weekday On Call RN

 

Monday - Thursday 4:30pm - 8:00am -  With Holiday Rotations

 

Territory will be the Harrisburg, Lancaster and York areas

 

Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a licensed, professional Weekday On-Call Nurse to provide in-home services, interventions and supportive care for weekends, holidays, evenings and overnight on-call duties. Due to the nature of this position, the On-Call Nurse must be flexible, exhibit a keen ability to work independently, execute sound judgment and successfully troubleshoot for a host of medical and non-medical situations.

 

Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. Responsibilities

  • Assuming responsibility for evening and night on-call duties
  • Assuming primary responsibility for total care census, including assessing, planning, implementing and evaluating all phases of the nursing process
  • Communicating with attending physicians, Clinical Coordinator, families, caregivers, and other community resources to report changes and coordinate optimal care/use of resources for patients and families
  • Attending monthly interdisciplinary team meetings to maintain up-to-date patient records, troubleshoot problems, sets plans and action goals and work in tandem to adjust care plans
  • Obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status
  • Monitoring physical care of patients and educating family, caregivers and volunteers in proper, approved techniques

Qualifications

Candidates must have a current RN license to practice in Pennsylvania and a valid drivers’ license.
