Part Time - State College Area


Integrated care leader Grane Hospice Care is expanding its core interdisciplinary team, and seeks a hardworking, compassionate and dedicated Certified Nursing Assistant (CNA) to join its ranks. This position will be responsible for traveling throughout the area and must have reliable transportation and a valid PA driver's license with current auto insurance.The CNA will be responsible for a host of critical day-to-day patient-focused tasks, from personal care and hygiene to general aid, assistance, personal support and much more. Because of the nature of this work, extreme empathy, commitment and a desire to help others is essential.



As a paraprofessional member of the interdisciplinary team, the CNA will work under the direct supervision of an RN, performing many services as required by individual patients. Day-to-day responsibilities include: 

  • Performing personal care needs as assigned, and tending to all tasks promptly and with the utmost care and attention to detail
  • Providing hygiene/personal care services including baths, back rubs, oral hygiene, shampooing and changing bed linens
  • Assisting in dressing and undressing patients, as assigned
  • Assisting in feeding patients, as assigned
  • Providing assistance with bedpans and urinals as well as light laundry, as needed
  • Assessing patients’ skin and providing proper care to prevent a breakdown tissue over bony prominences
  • Observing changes in patients’ conditions and reporting those changes to the RN Case Manager
  • Assisting in ambulation and exercise, as instructed by the RN Case Manager
  • Providing respite for families and caregivers when onsite
  • Ensuring patients’ living areas are clean and orderly
  • Adhering to all organizational documentation and care procedures and overarching standards of personal/professional conduct
  • Ensuring all patients’ immediate safety is tended to, including proper use and storage for tools and equipment


Applicants should be at least 18 years of age and a state-certified CNA. Ability to follow written and verbal instructions as well as document care provided is essential, as is basic computer/office competency. All candidates are expected to understand the general hospice and home health philosophies and be comfortable providing direct care to the terminally ill.


Since travel to patients’ homes is required, a valid driver’s license and reliable mode of transportation is also mandatory.
