This role will work directly with Grane’s elderly in-home population, devising and executing cutting-edge care plans for patients. Overall, the Occupational Therapist will be responsible for delivering the highest quality therapy services, in keeping with established plans of care. The focus of all treatments will be on improving patients’ self-reliance and ability to perform essential tasks in their own home environments. Core tasks include:

  • Evaluating the occupational therapy rehab needs of patients and preparing a care plan under physician’s orders
  • Maintaining a weekly schedule of patient visits, and delivering skilled therapeutic services to patients, in keeping with an established plan of care
  • Devising therapeutic activities and self-care activities designed to restore function
  • Initiating diagnostic, preventative and rehabilitative procedures based on ongoing assessments and patient evaluations
  • Identifying patient changes and emergent needs, and reporting observations/recommendations to physicians
  • Determining alternative techniques for daily living tasks and instructing patients, as needed
  • Initiating home programs for patients, families and/or in-home caregivers, including ongoing training and education, as needed
  • Making referrals to internal and external service providers, as needed
  • Carrying out billing tasks, including service- and time-based codes, RUGS, MDS rules and other necessary items
  • Discharging patients in accordance with established goals, plan of care and agency policies/procedures


The Occupational Therapist will be expected to demonstrate, through practice and documentation, strong knowledge of human physiology, pathology and diagnosis related to the discipline-specific aspects of ICD-9. All candidates should be graduates of an accredited baccalaureate school of occupational therapy, with a current state license to practice and registration with the American Occupational Therapy Association. Additionally, applicants should have 1-3 years’ experience as an occupational therapist in a home care setting, plus current CPR certification and a valid driver’s license. A master’s degree in occupational therapy and McKesson software experience are preferred, but not required.
