Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks an experienced, compassionate Licensed Practical Nurse to join its clinical personnel in delivering quality health/social services to the center’s diverse patient base. This role requires significant hands-on work with an elderly and terminal population, so the utmost empathy, flexibility and dedication are essential.


Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. Under the supervision of the RN Case Manager, the LPN will provide high quality nursing care to in-center and in-home patients, as prescribed by the physician and approved care plan. Because of the nature of Grane Hospice Care, the LPN’s responsibilities will be diverse and change based on the overarching needs of the organization and of the individual patients. These tasks include:

  • Ensuring high quality and safe delivery of hospice services to all patients, caregivers and families
  • Implementing care and other services following a thorough patient assessment and approved plan initiated by the RN Case Manager
  • Providing personalized pain/symptom management based on patient evaluations and responses to existing treatments/medications
  • Providing accurate and timely documentation of all patient services to reflect approved care plans
  • Training patients, families and caregivers on relevant aspects of the care plan to maximize self-sufficiency and comfort
  • Observing changes in patients’ conditions and reporting those findings to the clinical supervisor or interdisciplinary team members


All candidates must be graduates of accredited practical nursing programs, with current LPN licenses to practice in Pennsylvania. Additionally, candidates should have three or more years of nursing experience, with community health, hospice, medical and/or surgical work preferred. Candidates must be CPR certified.


Since some travel to patients’ homes is required, a valid driver’s license and reliable mode of transportation is also mandatory.
