Hospice Community Liaison

Beaver County Area

 

Due to a recent promotion, Grane Hospice Care is currently accepting applications from candidates that are motived, organized, and inspired to achieve results for a Community Liaison opportunity covering Dauphin and Lebanon Counties. The focus of this position is making connections with people which requires strong initiative and self-direction

 

A poised, engaging, and empathetic communication style based on natural warmth and enthusiasm is the key to achieving the goals of this position. The environment is fast paced and results oriented. Knowledge and skill in how to successfully influence and persuade others by understanding how their individual needs and motivations link is essential. The job requires a high degree of “selling” services in the marketplace. A self-confident, extroverted style that can enliven, engage and positively impact individuals and groups is essential.

 

The position has variety of tasks and is dynamic and changing. The position requires regularly meeting and pro-actively establishing relationships with new groups as goals and desired results can quickly change. The ability to track and effectively document into an electronic customer relationship management system is required daily.

Responsibilities

  • Identifying and collaborating with referral sources
  • Interpreting hospice benefits and services to the medical community and relevant professional groups
  • Developing and revising education programs for the community and prospective referral sources
  • Reporting patient concerns to appropriate staff and clinicians
  • Providing assistance to referral sources to promote and maintain continuity of quality patient care to hospice patients and families
  • Communicating Grane’s services and programs to agency referral sources, as requested
  • Participating in the monitoring and evaluating of Grane’s strategic plan or quality improvement, as requested
  • Participating in appropriate agency meetings, as requested
  • Assuming additional responsibilities and special projects as assigned by the Director of Business Development

Qualifications

Ideally, candidates will have a minimum of 3-4 years of varied relevant work experience and be able to demonstrate the ability to communicate effectively with members of the hospice interdisciplinary team and the community. Heavy local travel is required and all candidates must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
