Hospice care center seeks an Ordained CHAPLAIN/BEREAVEMENT COORDINATOR for meaningful in-center/in-home work

Full Time

Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a compassionate, flexible and open Chaplain to join its Chaplain & Bereavement Services department. This position requires ecumenical thinking and counseling, plus an ability to communicate with and relate to a diverse population of patients, families and caregivers during the inherent high stress periods associated with hospice care.

Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. Our mission is to provide premium comfort care to patients, families, and caregivers through an interdisciplinary team approach that understands, respects, and meets individual needs. As an organization, we strive to provide our patients, families, and caregivers with the highest level of satisfaction through our strong clinical outcomes and innovative approach by maintaining our core values of devotion, honesty, dignity, and respect.

Day-to-day responsibilities include:

  • Providing spiritual support to patients and families, based on their specific desires and belief systems
  • Working with staff, clergy and community groups to enhance their sensitivity towards the spiritual concerns of patients/families experiencing terminal illness and loss
  • Officiating and/or participating in funeral services for patients, as requested
  • Implementing and coordinating all spiritual service activities, and connecting those services to relevant patients and families
  • Integrating appropriate outside clergy and spiritual leaders, as needed by patients/families
  • Coordinating, planning and implementing community outreach services
  • Maintaining accurate patient visit records
  • Performing occasional liturgical assignments
  • Providing spiritual counseling to Grane staff, as requested
  • Creating, developing and implementing new ideas to promote strong community relationships and support

All applicants should be graduates of accredited seminaries/schools of theology, or relative Bachelor degree or possess appropriate certification in hospital or pastoral ministry. Additionally, a valid driver’s license and reliable mode of transportation is required, as this role will require some travel to visit in-home patients and families.
