The Physical Therapy position requires a variety of day-to-day and ongoing tasks and responsibilities, including:

  • Initiating diagnostic assessments and new patient evaluations, and developing specific care plans and therapeutic services 
  • Maintaining a weekly schedule of in-home visits to aid and assess patients
  • Carrying out in-home diagnostic, preventative and rehabilitative processes  
  • Treating patients to relieve pain, develop and restore function and maintain maximum performance, using diverse treatments, modalities and procedures
  • Providing ongoing evaluations and assessments to gauge patient progress and revised needs
  • Observing and reporting changes in patients’ conditions, and adjusting care plans accordingly
  • Assessing home environments and programs with patients, families and caregivers to promote family-centered therapeutic practices, and train participants as needed
  • Aiding in billing services, including time-based codes, RUGS, MDS rules and other standard practices
  • Making referrals to internal and external care providers, as needed
  • Holding ongoing caregiver and family meetings to discuss patient care and treatment plans
  • Ordering and/or fabricating adaptive/assistive devices and durable medical equipment, as needed
  • Supervising Physical Therapist Assistants and aiding in their growth and development


This role requires significant, hands-on work with an elderly population, so in addition to having direct geriatric experience, compassion, patience, flexibility and the ability to teach is essential to this role. All candidates should have a B.S. degree in a relevant field, plus the successful completion of an accredited physical therapy school. Current licensing and CPR certification is required, as is a valid driver’s license and 1-3 years’ experience in an in-home setting. A master’s degree in physical therapy and McKesson software experience is preferred, but not required.
