Compassionate, Hands-On NURSE PRACTITIONER Sought for Integrated Hospice Care Work

Full Time - Harrisburg Area

Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a Nurse Practitioner to join its ranks. This is an essential role within the clinical department, performing a variety of medical services to maximize patient care, comfort and overall satisfaction. This senior-level role reports to the center’s Director of Clinical Operations.

Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. Our mission is to provide premium comfort care to patients, families, and caregivers through an interdisciplinary team approach that understands, respects, and meets individual needs. As an organization, we strive to provide our patients, families, and caregivers with the highest level of satisfaction through our strong clinical outcomes and innovative approach by maintaining our core values of devotion, honesty, dignity, and respect.

The Nurse Practitioner will be responsible for a host of clinical and supportive tasks throughout Grane Hospice, including:

  • Conducting face-to-face visits to determine eligibility/continuing eligibility for hospice services and preparing concise, legible, timely clinical notes utilizing agency guidelines.
  • Performing clinical care including bedside evaluations, histories, physical examinations, radiological/laboratory interpretations, and general case management

  • Working with physicians and other care providers to optimize patient care, and providing written reports to primary medical staff, as requested

  • Observing and reporting significant changes in patients’ conditions, then consulting with clinical staff, as needed

  • Prescribing medication to the extent permitted by the Nurse Practitioner’s license

Because of the nature of hospice, extreme compassion, empathy, flexibility and patience must be exhibited at all times. Applicants should have an appropriate master’s degree, plus be actively licensed to practice as a nurse practitioner in Pennsylvania. All candidates must have at least one year of practical experience with a strong working knowledge of hospice philosophy and medications. Travel is required for this position, so a valid driver’s license and reliable mode of transportation is also essential.
